An Appointment Confirmation e-mail will be sent to you. I will call you to book an alternate time if there are any conflicts. Cancellations cannot be made online. Please call to cancel an appointment.

If you are a new patient please provide a telehone number and I will call you with information for your first appointment. Initally, please book several appointments two or three weeks apart to enable treatment progression. The appointments can be cancelled later if not required.
